伴隨人口老化速度激增,所有人都有發展衰弱的風險,然而衰弱的定義和概念化缺乏共識,以及越來越多的衰弱評估工具,如何選用衰弱篩檢工具及判定衰弱狀態是一項挑戰。本研究目的為(1)以系統性文獻回顧探討社區老人衰弱篩檢工具之心理測量屬性,(2)比較三種衰弱篩檢工具之心理測量屬性、社區老人衰弱狀態隨時間進展之轉換變化及衰弱篩檢之可行性,並找出最佳預測社區老人衰弱之函數,與社區老人衰弱狀態對於跌倒、住院、死亡發生預測情形。採用前瞻性世代研究(prospective cohort study)設計,分層便利取樣,共收集110位受試者,於基期、第六個月、第12個月不同時間點重複測量,以衰弱身體表型(FP)、骨

質疏鬆性骨折研究(SOF)及介護預防篩檢(KCL)工具及受試者基本屬性資料表進行資料收集。收集時間為110年3月至111年3月。資料以SPSS 20.0版進行分析,包含描述性統計及獨立樣本t檢定、卡方檢定、單因數變異數分析、重複測量變異數分析、生成接收者工作特徵曲線分析、逐步迴歸判別分析等推論性統計。經系統性文獻回顧納入58篇文獻評析,合計有25種衰弱篩檢工具,發現多數衰弱篩檢工具僅呈現一種信度或效度,心理測量屬性以效標關聯效度佔最多、內在一致性為其次。25種工具中,有13種工具有使用標準工具做比較進行ROC分析,依據AUC、心理測量屬性及受使用廣度選出本研究第二階段進行縱貫性研究之工具為KC

L、SOF及FP。縱貫性研究顯示,基期以衰弱身體表型(FP)測量衰弱狀態與婚姻狀況、獨居與否、居家環境、自覺健康情形、主要經濟來源、轉介來源達統計上顯著差異。由基期、半年及一年三次篩檢結果發現,SOF篩檢結果與FP篩檢結果相較呈現較多的穩健,若需要早期發現衰弱傾向,則FP工具較勝任。KCL篩檢工具包含身體、心理及社會構面,與SOF及FP偏向身體構面不同,相較之下可能高估衰弱的存在。一年內衰弱狀態之間的轉換,變好及變差之改變約三成至四成,有六成至七成受試者於半年及一年時維持原有衰弱狀態。以SOF工具測量,於基期至半年,衰弱狀態轉換與教育程度、婚姻狀況、獨居與否達統計顯著差異;以KCL工具測量,於

基期至一年,衰弱狀態轉換與年齡與性別達統計顯著差異。逐步迴歸判別分析於基期、半年及一年之判別預測變項可由原始10項分別下降為4項、2項、2項,分別為(1) 體重減輕、下肢功能下降、降低精力及日常生活活動能力,(2) 日常生活活動能力及牙口功能,(3) 身體功能及牙口功能。預測基期、半年及一年FP篩檢結果,發現KCL之牙口功能構面預測衰弱之準確度有達七成四。KCL工具之內在一致性信度高,而SOF及FP之內在一致性信度低。KCL、SOF、FP三種衰弱篩檢工具之專家效度皆佳。三種衰弱篩檢工具中以SOF完測率最高、FP次之,KCL完測率最低。測量所需時間以SOF最短,KCL次之,FP最長。本研究結果可

目的:探討衰弱前期及衰弱期老人執行12週抗阻力及腳趾抓握運動介入成效探討。假設:以衰弱前期、衰弱期兩族群老年人,介入長達12週,每次60分鐘,每週2次共24次抗阻力、短足運動(short foot exercise)及腳趾抓握運動,評估後測結果,來瞭解改善其下肢肌力、平衡、生活功能獨立執行測量的情形。方法和研究設計:本實驗為單一介入的實驗,社區中招募兩組受試者據點組14位長者(衰弱前期11人,衰弱期3人),日照組12位長者(衰弱前期1人,衰弱期11人)。兩組行抗阻力、短足運動、腳趾抓握運動長達12週,並前後測,使用簡式伯格量表(SFBBS)測試平衡、手持肌力測試儀(HHD)測試髖屈肌(Hip

flexors)、股四頭(Quadricep)、足部背屈(Ankle dorsiflexion)、足背蹠屈(Ankle planter flexion),大拇趾(Hallux)、小趾肌力(Lesser Toe),生活功能量表(FIM)成效。結果:研究結果顯示以衰弱前期組12位受試者及衰弱期組14位受試者呈現數據結果。衰弱前期組身體組成肌肉量及內臟脂肪有顯著改善,平衡能力P-value<0.05,肌力Ankle dorsiflexion、Ankle planter flexion、Lesser Toe:P-value<0.05,Hallux:P-value<0.01。而衰弱期組在平衡能力P-v
